Items 1 - 12 are important for admin purposes and should be completed by those sponsoring animals for this event, if possible.
Items 13 - 21 are not crucial for admin purposes, but give us information which may be relevant to the treatment. Include the info you are able to send us, and which you feel may be relevant for each animal at this time. All information is treated in confidence.
