JOB SUMMARY
Position in the New Options Traumatic Brain Injury Demonstration Project based at a non-profit Center for Independent Living, to provide services that promote independent living to persons with traumatic brain injuries (TBI) who are trying to live more independently within their communities. Position will provide services throughout Santa Cruz County.
SIGNIFICANT DUTIES
CONSUMER SERVICES
Provide the following independent living core services to survivors of traumatic brain injuries (TBI): information and referral, individual advocacy, personal assistance services, housing assistance, peer support, and independent living skills training. Core services are provided in the office through in person and telephone contact and in the community as assigned.
Provide the following traumatic brain injury core services to survivors of traumatic brain injuries (TBI): intake, assessment, and individual service plan development, service coordination, community reintegration services, supported living services, and vocational supportive services. Core services are provided in the office through in person and telephone contact and in the community as assigned.
Teach independent living skills, such as organizational skills and time management, toTBI survivors. Train family of participants to provide ongoing support in these areas.
Provide case coordination services as requested by consumers and/or as required by the New Options funding source, in conjunction with other New Options Consortium members.
Be available in the office to interact with consumers and co-workers regarding independent living issues, and to provide relief coverage in the absence of other co-workers.
Convey the independent living philosophy and principles of consumer control, equal accessand equal opportunity, throughout delivery of services.
FILE MAINTENANCE, DATA COLLECTION, & REPORTING
Create and maintain confidential files on consumers who request ongoing services:complete intake assessment, prepare case notes, document service requests and services provided,complete periodic statistical data collection forms, and complete other paperwork and reporting as required.
Maintain monthly follow-up contact and documentation for all consumers who have an open file.
Keep daily logs and prepare monthly and quarterly statistical information in an accurate and timely fashion as required by the agency and/or funding source(s).
OUTREACH & EDUCATION
Provide educational forums and presentations on topics related to independent living and traumatic brain injury to participants, caregivers, and professionals in the community.
Provide outreach activities to meet the cultural and ethnic needs of the population with TBI in the geographic area served.
Participate in community events with the purpose of conducting outreach to potential New Options participants.
Participate in community education activities that promote awareness of the issues faced by survivors of traumatic brain injuries.
Facilitate support groups for individuals with TBI and document case notes and service units for New Options participants attending the support group(s).
Assist in the identification and documentation of service needs and the development of necessary programs and services to meet the needs of individuals with TBI in the geographic area served.
Collect, read, summarize and disseminate information on local resources, issues, and legislation relevant to persons with traumatic brain injuries.
COLLABORATION & NETWORKING
Work closely and coordinate with organizations in the service area serving persons with TBI, their caregivers and families in order to ensure that the greatest numbers of persons are served and that the greatest numbers of organizations participate.
Develop and maintain a consortium or network of community agencies with an interest in survivors of traumatic brain injury that serves as an effective referral system, case conferencing system, and ongoing forum for professional education, training and outreach.
Facilitate consortium monthly meetings: create an agenda and take minutes, and send out minutes and agendas for such meetings.
Facilitate consortium case conferences and update participant files with information from such case conferences.
Attend assigned trainings and meetings.
OTHER DUTIES
Follow CCCIL policies and procedures.
Maintain practices that protect the safety and health of consumers and co-workers.
Provide own transportation throughout service area, including areas not served by public transportation.
Be available to work occasional evenings and weekends.
Operate a keyboard and word processing program on a computer.
Operate standard office equipment (including fax machine, copier, and Telecommunications Device for the Deaf (TDD)).
Perform other duties as assigned.

EXPERIENCE/KNOWLEDGE REQUIRED
Demonstrated knowledge of:
Santa Cruz County, includingcommunity resources, and disability, cultural, and immigrant issues
Issues faced by survivors of traumatic brain injuries and of community resources available
Chemical dependency and recovery issues
Demonstrated skills and experience in:
Promoting understanding of brain injury concerns to diverse audiences
Communication with persons of diverse backgrounds and communication styles
Informal teaching
Organization, time management, and prioritization
Working independently and as part of a team
Statistical data collection and reporting
General office procedures and office equipment
EXPERIENCE/KNOWLEDGE PREFERRED
Bilingual: English/Spanish
Work or educational experience in the social services field
Personal experience with disability
Familiarity with independent living philosophy and principles

Ability to present information and ideas clearly both orally and in writing in English, and to understand and retain information and ideas presented orally and/or in writing
Ability to protect the confidentiality of consumer information
Ability and willingness to work with a wide range of people with and without disabilities, including people in crisis
Sensitivity to issues concerning diverse cultures and people with a range of disabilities
Ability to establish and maintain friendly and professional working relationships with traumatic brain injury survivors and family members, the general public, and agency personnel, in both group and individual interactions
Ability to take initiative and, after completion of initial training, ability to work away from the office and with limited direct supervision
WORKING CONDITIONS
The emotional effort varies but can be challenging. Occasionally there are short deadlines and periods of high activity/demands. The job entails interacting with persons with a variety of disabilities and a variety of communication and behavior styles. Some consumers are facing crisis situations.
JOB SETTING
CCCIL is co-located with the Department of Rehabilitation located at 1350 41st Avenue, Suite 101 in Capitola. CCCILThere are 4 CCCIL employees who work within this office.Office space is shared, two private offices and two cubicles.There will be periods of time in the office and at meeting places throughout Santa CruzCounty.The Capitola office is physically accessible, and is on public transportations lines.
PHYSICAL DEMANDS
Within the office the job is primarily sedentary with some movement required to the file cabinets, copier, and fax machine. Some consumer appointments, meetings, workshops, outreach, and events are conducted outside the Capitola office throughout Santa Cruz County, including some locations which are not accessible by public transportation and which may not be physically accessible.
COMPENSATION AND BENEFITS
Salary: $2,600.00 per month

Sick Leave: Accrues at a rate of 3.75 hours per pay period
